Badalona Serveis Assistencials (BSA):
Badalona Serveis Assistencials is an integrated healthcare organization, which was set up in order to provide effective assistance and to respond to peoples’ needs in the territory of Barcelonès Nord (north of Barcelona urban area), including Badalona (a seaside town that lies 6 miles away from Barcelona). More than a thousand professionals provide services to a population of over 435,000 people in several settings: one acute hospital, one intermediate care hospital and seven primary health-care centers.

National Yang-Ming University Hospital

The Integrated Hip Fracture and Osteoporosis Rehabilitation Program of Elderly in Yi-Lan (i-HOPE) was launched in February 2014. It is designed specifically for patients aged above 65 years who have suffered a low-impact-trauma hip fracture. Combining orthopaedic medicine, surgical care and rehabilitation, our practice includes nutrition consultation, medicine reviews and household environmental assessment. By setting individualized treatment plans and goals, it aims to provide multi-dimensional care and instructions to prevent the next fracture.

Durham VA Medical Center

The Durham Veterans’ Affairs (VA) Medical Center is located in Durham North Carolina in the U.S. and serves a population of around 364,000 Veterans. The Bone Health Service started in the fall of 2011 identifying patients with fractures using ICD9 codes and screening those over age 50 for fragility fractures in order to recommend NOF treatment guidelines to providers. The fall of 2013 saw the addition of a Bone Health Nurse to provide patient education and to coordinate care between provider and patient. The Bone Health Service serves about 422 Veterans per year.

Whangarei Hospital

Health New Zealand - Te Whatu Ora. Health New Zealand - Te Tai Tokerau, is a public hospital system with a network of four hospitals serving more than 184,000 people. The Fracture Liaison Service is in the Directorate of Medical and Elder Services and manages all fragility fractures (including hip, inpatient, outpatient and vertebral fractures). Approximately 800-850 fracture patients are assessed by the FLS per year. 

Maxima Medical Centre

Based in Holland, the Dutch Maxima Medical Centre started its Fracture Liaison Service programme in 2004 with a mission to ‘Stop At One: Making your first fracture your last’. This programme is a collaboration among an endocrinologist, a nurse that specializes in fracture care, a nurse practitioner, orthopedic surgeons and each patient’s general practitioner.
