Every patient with an acute fracture presenting to the orthopedic services, is evaluated by the FLS team. The FLS team consists of Geriatrics and Endocrinologists. A comprehensive assessment is done evaluating risk factors for osteoporosis and a fall risk is made. All patients are evaluated and treated acutely with Intravenous bisphosphonate therapy. They are then followed up within 3 months and then further managed chronically at the combined osteoporosis clinic.

We provide extensive support by involving physicians, nurses, pharmacists, rehabilitation, and administrative staff in the team to address/treat fragility fractures,  assess fracture risk,prevent new secondary fractures.

In addition, we have established an outpatient osteoporosis clinic and provide personalized treatment for each patient in a specialized outpatient clinic led by an osteoporosis manager.

Fracture liaison service ensure fracture risk assessment, and treatment where appropriate, is delivered to all patients with fragility fractures.

 In 2019 an FLS was implemented in a small hospital (235 inpatient beds) that is the only hospital in the region.

当院では、2018年4月にOLSチームを立ち上げ、骨粗鬆症対策に多職種で取り組んできました。現在は、骨粗鬆症委員会として活動を継続し、昨年は「きそがわ骨粗鬆症デー」を開催し、地域住民への啓発を実施。また、職員向けには各専門分野による研修を輪番制で年1回行い、知識向上を図っています。外来では薬剤師外来を開設し、医師と連携して治療導入指導を実施。自己注射指導は薬剤師と看護師が連携。放射線技師はCT画像を再構築し椎体骨折の診断支援を行い、リハビリでは転倒リスク評価と運動療法・環境調整により再骨折予防に努めています。

Orthogeriatric liaison service was initiated in Sarawak General Hospital November 2022. This service was initially suggested by our hospital director who saw the potential to improve the quality of hip fracture care in the hospital. We will first aim to educate and build up the knowledge of our core team member and focus on improving the current hip fracture care and services in elderly population. Our aim is to grow and sustain our fracture liaison service. 

山口県北浦地区の総合病院として急性期から慢性期の疾患を対象としています。山口県は高齢化率が約34％であり特に長門医療圏では高齢化率44％であります。この傾向は今後も続いておりそれを診療する医師も少数であり、二次性骨折予防への取り組みは今以上に重要であると考えます。
