Fracture Liaison Service based within the community of Middlesbrough, Redcar and Cleveland in the UK.

Service involves: 

- Identification of any low trauma fracture in those aged 50 and over in the agreed catchment area.

- Completion of bone health and falls assessment, referral for DXA, diagnose and recommend treatment.

- Monitoring medication compliance at 4 month and 12 month.

- Pathways in place to refer to other community services and specialist bone clinic amongst others as needed.

- Strong links with the community Falls Service.

Ohara General Hospital is located in the center of Fukushima City. As a regional core hospital, we accept many patients with fragility fractures, such as proximal femoral fractures, spinal fractures, and limb fractures. We have built Fukushima Osteoporosis Medical Network with 110 clinics in Fukushima City (http://www.f-ishikai.jp/examination/3943.html), and based on that, we are working to prevent primary fractures through osteoporosis screening and secondary fractures after fractures.

The FLS at our center aims to identify, assess, and treat patients with osteoporosis and other fracture risk factors through a multidisciplinary team of over 20 specialists from orthopedics, endocrinology, geriatrics, and rehabilitation, among others, with the goal of reducing the risk of subsequent fractures.

The UZ Gent fracture liaison service is coordinated by the Unit for Osteoporosis & metabolic bone disease in collaboration with the reference centre for rare bone, calcium and phosphate disorders.
