FLS en un hospital universitario de IV nivel de complejidad en donde capturamos las fracturas vertebrales radiológicas y clínicas por fragilidad en mayores de 50 años y las fracturas de cadera por fragilidad en mayores de 65 años por el servicio de endocrinología y ortogeriatria respectivamente, para ser estudiadas y tratadas de forma apropiada. 

当院では多職種が強固に連携し、地域に根ざした二次性骨折予防を実践しています。2012年から継続する地域住民向け講演会は計22回、延べ487人が受講し、疾患啓発に大きく寄与しています。

実臨床においては、地域連携を通じて脆弱性骨折患者を積極的に応需し、48時間以内の早期手術や新規治療導入時には全職種が一体となった指導体制を構築しています。外来でのオスタバロ等（自己注射製剤）の開始に際しても、医師の要請に基づき薬剤師が個別指導を徹底し、理解度とアドヒアランスの向上を図っています。

さらに、薬剤関連顎壊死（MRONJ）のリスク低減に向けた医科歯科連携も当院の特徴です。リーフレットを用いた啓蒙から口腔管理に至る包括的アプローチにより、二次性骨折予防における質の高い医療を継続的に提供しています。

二次性骨折（再骨折）を予防するためのチームが、当院で2024年4月からスタートした。様々な専門職が協力して切れ目のないサポートを提供している。医師、看護師、薬剤師、リハビリ、放射線技師、MSW、事務など多職種が連携して、骨粗鬆症の適切な診断・治療や転倒予防を継続的に行っている。外来患者に対しても医師、薬剤師が治療をサポートを行っている。

The University Clinical Centre in Gdańsk is the primary teaching hospital of the Medical University of Gdańsk. It is a multi-specialty institution— the largest in northern Poland and among the largest nationwide. The Centre comprises the Invasive Medicine Centre, one of the most modern medical facilities in Europe, as well as the Non-Invasive Medicine Centre, which houses several of the hospital’s in-patient departments.

Het O.L.V. van Lourdes Ziekenhuis Waregem is een dynamisch regionaal ziekenhuis met 267 erkende bedden waarvan 48 G bedden. Preventie en behandeling van osteoporose wordt georganiseerd door de diensten geriatrie, fysische geneeskunde en endocrinologie waarbij de 75+ worden opgevolgd door de geriaters. De jongere patiënten worden ifv hun comorbiditeit geholpen door artsen fysische geneeskunde of endocrinologie. 

Er is een afdeling orthogeriatrie waar alle 75+ met een fragiliteitsfractuur (heup, bekken, humerus, pols ...) worden opgenomen. 

Hospital Ribera Povisa is part of the Ribera healthcare group and is the only private hospital in Galicia with an agreement with the public health system (SERGAS). It is a member of the Association of Galician Hospitals (AHOSGAL) and holds the Joint Commission International accreditation.

Our Fracture Liaison Service (FLS) is a multidisciplinary team comprised of rheumatologists, orthopedic surgeons, rehabilitation specialists, a case manager nurse, and an orthogeriatrician, serving a catchment population of 113,000 people.

The Sahlgrenska University Hospital Mölndal coordinator based FLS was established in 2014 and now includes patients aged 50 years and older with any clinical fracture. All clinical fractures are identified using the Swedish Fracture Register. AI-based screening on X-ray reports is used to identify patients with vertebral fractures not captured in registers. Nearly 6000 fracture patients are identified yearly.
