Filière fracture reconnue par le GRIO, créée depuis janvier 2023, CPAFO est portée par des gériatres. Elle dépiste l'ostéoporose chez les patients fracturés et chuteurs passés par le centre hospitalier (hospitalisés et non hospitalisés). Grâce à la collaboration du service informatique et de la radiologie, tous les patients de plus de 65 ans bénéficient d'une analyse de leur passage par une infirmière coordinatrice selon un algorithme décisionnel (localisation fracture, chute, âge).

Our service is run with the collaboration of Te Whata Ora NZ and ACC NZ. We provide the FLS service where we receive referrals, assess the clients for falls risks, complete DEXA scans and reports and also complete strength and balance classes, education to clients and their families, and provide follow ups with clients and their families at 16 weeks and 52 weeks post fracture
