What is a Post Fracture Care Coordination Program ?

Post-Fracture Care (PFC) Coordination Programs, such as Fracture Liaison Services (FLS), are coordinator-based, secondary fracture prevention services implemented by health care systems for the treatment of osteoporotic patients.

PFC Programs are designed to:

Close the care gap for fracture patients

80% of whom are currently never offered screening and/or treatment for osteoporosis

Enhance communication between health care providers

by providing a care pathway for the treatment of fragility fracture patients

Best Practice Framework

The Best Practice Framework (BPF) sets an international benchmark for FLS and serves as the measurement tool for IOF to award ‘Capture the Fracture Best Practice Recognition’ in celebration of successful FLS worldwide.

Join the community and have your FLS mapped on the CTF map of best practice page.

Mentorship

The CTF mentorship programs offer opportunities for all PFC programs, such as Fracture Liaison Services to learn from experienced PFC program champions.

Implementing Strategies

Fracture Liaison Services (FLSs) are recommended worldwide to reduce fracture risk after a sentinel fracture.

The IOF Capture the Fracture working group and the Capture the Fracture® team are actively involved in creating new educational programs and resources to help FLS implement sustainable and effective services.

Visit the Implementing Strategies page to learn more about Coordinator-based systems for Secondary Prevention in Fragility Fracture Patients, the 11 Key Performance indicators to measure the effectiveness of fracture liaison services and how to effectively identify fragility fracture patients.

Resource Center

The Resource Center is a dedicated library for all our PFC and CTF related materials. Our materials include a wide range of valuable tools and resources to help health care professionals will all facets of setting up and developing sustainable PFC programs.

Call to action

A consensus has developed across the world on the pressing need for fragility fracture sufferers to receive the secondary preventive care that they need. Visit our calls to action page to view publications from international groups and champions in several countries which demand widespread implementation of PFC programs
