Our Fracture Liaison Service (FLS) delivers a comprehensive, patient-centered model designed to systematically identify, assess, and treat individuals aged 50 and above who present with fragility fractures. The service operates through a fully integrated pathway that ensures >90% case finding coverage, rapid assessment, and timely initiation of secondary fracture prevention.

 

All patients are proactively identified through daily orthopedic and radiology surveillance, followed by standardized risk assessment using DXA, vertebral fracture assessment, laboratory evaluation, and FRAX®-based stratification. Treatment recommendations follow evidence-based national and international guidelines, with therapy initiation consistently documented and communicated to primary care for long-term continuity.

 

Our pathway includes structured follow-up at 3 and 12 months to ensure treatment adherence, monitor outcomes, and support patient education. Continuous quality improvement is embedded through monthly audits, performance dashboards, and multidisciplinary team meetings. Key performance indicators consistently meet IOF Capture the Fracture® Gold criteria, reflecting high rates of identification, assessment completion, treatment initiation, and adherence monitoring.
