POK OI HOSPITAL

  The Fracture Liaison Service (FLS) at Pok Oi Hospital (POH) is a specialized program dedicated to elderly patients who have suffered from hip fractures. Its primary goal is to provide comprehensive and coordinated care to optimize patient outcomes, minimize the risk of future fractures, and enhance overall quality of life.

  The FLS team at POH includes orthopedic surgeons, geriatricians, nurses, physiotherapists, occupational therapists, prosthetic & orthotic therapists, dietitians, and medical-social workers, working collaboratively to address the unique needs of geriatric patients through a holistic approach.

  Services include early identification and assessment of individuals at risk for secondary fragility fractures, thorough medical evaluations, effective pain management, surgical interventions if necessary, rehabilitation programs, and education on fall prevention and bone health.

  A key priority is ensuring continuity of care, facilitating smooth transitions for patients from the hospital to rehabilitation centers or their homes, and collaborating with community resources for ongoing support and follow-up care after discharge.

  By leveraging multidisciplinary expertise and evidence-based practices, the FLS at POH plays a critical role in reducing the negative impact of hip fractures on geriatric patients, aiming to lower morbidity and mortality rates associated with such fractures and improve overall well-being.
