Our service identifies patients aged 50 and over who present with fragility fractures and ensures they receive appropriate osteoporosis assessment and management. Patients are reviewed for bone health, fracture risk, and fall risk, and are provided with education and treatment recommendations where needed.
We work closely with hospital departments and primary care providers to support ongoing care and follow-up. The aim of the service is to improve osteoporosis care, reduce the risk of future fractures, and support patients in maintaining their bone health.
