TH1 FLS — Thonburi Hospital, Bangkok, Thailand

Thonburi Hospital's Fracture Liaison Service (TH1 FLS) was established in 2017 under the leadership of Prof. Aasis Unnanantana (Orthopedic Surgery) and coordinated by an experienced orthopedic nursing team. The service primarily focuses on hip fracture patients, with 186 fragility fracture patients managed in the most recent 12-month period, including 98 hip fractures.

Our multidisciplinary team comprises orthopedic surgeons, nurses, and physiotherapists who deliver comprehensive post-fracture care. Key strengths include 24-hour orthopedic surgeon availability, urgent hip surgery within 48 hours (94% rate), and on-site DXA for bone density assessment.

All hip fracture patients undergo systematic evaluation including DXA, blood tests for secondary causes of osteoporosis (calcium, phosphate, vitamin D, parathyroid hormone, liver and renal function, bone markers), falls risk assessment, and lifestyle risk-factor screening. Over 95% of patients receive osteoporosis treatment, medication review, and falls prevention interventions. Treatment decisions follow national guidelines using FRAX risk assessment.

Patients are re-evaluated within 6 months, with outcomes communicated to primary care physicians, bone metabolic specialists, and the home visitation team, ensuring continuity of care beyond the hospital setting.
